- Technical Assistance:
High chairs should be cleaned & sanitize before & after meals/snacks. Staff should always sit with children during meals. Child with learning disability may need additional staff. Obtain documentation regarding needs.
An unannounced monitor inspection was conducted on July 24, 2015. Upon arrival at 8:55 am, there were twenty-eight (28) children ages 5 months ? 8 years old, the Owner, Center Director & six (6) staff members present. Attendance in each group was as follows: Infant Classroom There were six (6) children ages 9-14 months old & two (2) staff members present. Toddler Classroom There were four (4) children ages 16 months-23 months & one (1) staff member present. 2 Year Old Classroom There were seven (7) 2 year old children & two (2) staff members present. 3-4 Year Old Classroom There were five (5) 3-4 year old children & one (1) staff member present. School Age There were six (6) school age children & one (1) staff member present. Children records and records for recently hired staff members were reviewed. Center does not transport children & does not administer medication. Children were observed during arrival departure procedures, active/quiet activities, self-initiated and directed activities; bath room/diapering/hand washing procedures, meals/snacks served & nap time. There are no violations resulting from this complaint investigation. LI Anderson concluded inspection at 4:18 pm. Risk Assessment & areas of non-compliance found during today?s inspection were reviewed with the Owner & Center Director on 7/24/2015.
Standard #: 22VAC40-185-40-H Description: Based on record review, the Licensee failed to maintain public liability insurance. EVIDENCE: Certificate of Liability document shows an expiration date of 5/06/2015. Plan of Correction: Owner will renew or purchase required insurance
Standard #: 22VAC40-185-270-A Description: Based on inspection, the Licensee failed to keep areas and equipment maintained in a clean, sage and operable condition. EVIDENCE: In the toddler classroom, LI Anderson observed cords on horizontal blinds hanging low enough for the children in care to grab & long enough for it to be a strangulation hazard. Plan of Correction: Staff will hang cord so the children cannot reach. Corrected during inspection.
Standard #: 22VAC40-185-500-A Description: Based on observations, the Licensee failed to ensure staff hands are washed before assisting children with food. EVIDENCE: In the 2 year old classroom, LI Anderson observed 1 staff member assist a child with food served during lunch, without 1st washing here hands. Plan of Correction: Staff will wash hands after assisting children with meals/snacks. Center Director will review Center hand washing policies & procedures.
Standard #: 22VAC40-185-550-D Description: Based on record review & discussion with staff, the Licensee failed to implement & document a minimum of 2 shelter-in-place drills per year. EVIDENCE: Center did not implement at least 2 shelter-in-place practice drills within the past year Plan of Correction: Center Director will implement at least 2 shelter-in-place drills annually
Standard #: 22VAC40-185-560-K Description: Based on observations, the Licensee failed to ensure tables used for meals/snacks, are cleaned & sanitized before & after serving. EVIDENCE: In the 2 year old classroom, Staff did not clean nor sanitize 1 table used to serve lunch. Plan of Correction: Staff will clean & sanitize tables before & after each use.
