7/13/15 The Inspector was on site today after receiving a report from center management staff regarding infant sleeping practices. Staff interviews and observations in the classroom were conducted today. 7/15/15 Administrative documents were reviewed today. One violation associated with this reported was cited. Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records, it was determined that one record was not complete. Evidence: Staff record A does not include: emergency contact information; a TB Health Screening report.

Plan of Correction: I will have her complete the emergency contact form today. The TB test was misfiled, but is in file. [sic]

Standard #: 22VAC40-185-370
Description: Based on staff interviews, it was determined that one infant was not placed in his crib on his back. Evidence: During the course of this investigation, it was determined that on two separate occasions staff person A placed a 5 1/2 month old infant in his crib on his stomach. It was reported to the Inspector that this took place on Monday, July 6, 2015 and on one other day during the previous week. Staff person A indicated to the Inspector that despite the fact that the infant could not roll over on his own, she put him in the crib on his stomach because he sleeps better that way.

Plan of Correction: Staff was immediately retrained regarding safe sleep positioning. Written documentation was also redistributed as well as placed on walls above cribs as further reminder. [sic]

Standard #: 22VAC40-191-60-C-1
Description: Based on a review of background checks, it was determined that one Criminal History Record Check (CRC) was not requested within 7 calendar days of the staff person's date of hire. Evidence: Staff person A has a documented hire date of 4/24/15. Center management staff did not submit her CRC request within 7 calendar days of her date of hire. The CRC request was not submitted until July 2, 2015.

Plan of Correction: CRC has been requested. All other CRC requests will be made within 7 calendar days. [sic]
