An unannounced monitoring inspection was conducted on July 21, 2015. At the time of entrance there were 58 children and 9 staff members present with children. The sample size consisted of 3 children's records and 5 staff records. Three medications were observed. Children were observed preparing for afternoon nap, preparing for snack, reading books and playing with cars. Previous violations were reviewed. The inspection began at approximately 11:00 A.M. and ended at 3:00 P.M.. Areas of non-compliance are identified on the violation notice. The results of the inspection and violations cited were reviewed and verified with the center director during the exit interview on July 21, 2015. Please complete the columns for description of action to be taken and date to be corrected for each violation cited on the violation notice, and then return a signed copy to the licensing office within 10 calendar days of receipt. Standard #: 22VAC40-185-60-A Description: Based on a record review and staff interviews the licensee, did not ensure each child had a record containing all the required information. Evidence: During the inspection July 21, 2015, the licensing inspector reviewed 4children's records and determined the record for chid #1, #2 and #3 were incomplete as they were missing the following items; 1) The record for child #1 did not include the employment information for one parent listed 2) The record for child #2 did not include the home address for one parent listed and documentation of a physical on file within one month of attendance. First day of attendance for child #2 was 1-20-2014. 3) The record for child #4 did not include the employment information or the home address and home phone number for the second parent. Plan of Correction: All parents will be contacted today and corrections/missing info updated. Moving forward management will ensure that all required enrollmen information is completed prior to the first day of enrollment. DM will review for compliance quarterly.
Standard #: 22VAC40-185-90--A Description: Based on a record review and staff interviews, the licesee did not ensure that the record for each child has a written agreement between the parent and the center and it is in each child's record by the first day of the child's attendance. The agreement shall be signed by the parent. Evidence: During the inspection conducted on July 21, 2015 the records for child #1, #2, #3 and #4 did not include the parent agreement regarding a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately. Plan of Correction: Parent agreement forms were put together today to have all parents sign by July 24, 2015. All new enrollment packets will include this form and DM will review for compliance at quarterly school visits.
Standard #: 22VAC40-185-280-B Description: Based on observation and staff interviews, the licensee did not ensure all hazardous substances such as cleaning materials, insecticides, and pesticides are kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection conducted on July 21, 2015, in the infant room the cabinet directly under the changing table and accessible to children was unlocked and it contained 2 containers of disinfectant spray labeled "keep out of reach of children" and "caution". Plan of Correction: Staff will be retrained on or before July 24, 2015 on proper chemical storage. Management will observe daily for compliance.
Standard #: 22VAC40-185-500-A Description: Based on observation, the licensee did not ensure that all staff washed their hands with soap and running water before and after helping a child use the toilet or a diaper change. Evidence: During the inspection conducted on July 21, 2015 the licensing inspector observed staff #3 helping 3 different children in the restroom and changing their diapers and staff #3 did not wash their hands after assisting each child. Plan of Correction: Staff will all be retrained on proper diapering and handwashing policies on or before July24, 2015. Management will observe daily for compliance.
Standard #: 22VAC40-185-500-B Description: Based on observation, the licensee did not ensure that a nonabsorbent surface for diapering or changing shall be used for children younger than three years, this surface shall be a changing table or countertop designated for changing and that disposable diapers were disposed of in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: During the inspection conducted on July 21, 2015 the licensing inspector observed the following; 1) Staff #3 changing the diapers for child #1 and child #2 and each child was changed in the restroom and a diaper changing table or coutertop was not used. Both children are under the age 3 and staff #3 confirmed they only use the changing table when the children have bowel movements. 2) Staff #3 was observed using their hand after changing soiled diapers to lift the lid of the designated trashcan to dispose of soiled diapers and did not use a foot operated storage system so that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Plan of Correction: Staff will all be retrained on proper diapering and handwashing policies on or before July24, 2015. Management will observe daily for compliance.
