Technical Assistance:
Injury records and the need for detailed information regarding notification of parents and full given names of children/staff were discussed. Bottle preparation was discussed.

The inspector arrived at 1:37 PM and departed at 4:00 PM. Nap time, snack procedures and transition to afternoon activities were observed. A sampling of four children's records and six staff records were reviewed. Injury records were reviewed. The program director stated that there is no children's medication stored at the facility and no medication has been administered to the children in care. An exit meeting was discussed with the program director during which the facility's plans of correction were obtained prior to the closure of the inspection. Standard #: 22VAC40-185-130-B
Description: Based upon review of four children's records, the facility has not ensured that they obtain documentation of additional iimmunizations once every six months for children under the age of two years. Evidence: The facility was not able to provide documentation of additional immunizations since the most recent immunizations on record for child 2, born on 11/23/14, dated 1/26/15.

Plan of Correction: The facility responded with the following: The parent was contacted during the inspection and will bring in the updated immunizations that were obtained the end of June. A tracking sheet will be created for those children under the age of two to ensure that updates are obtained.

Standard #: 22VAC40-185-550-M
Description: Based upon review of injury records since April 2015, the facility has not ensured that they maintain a written records of children's injuries that includes all required information. Evidence: 1. The record of an injury to child 5, dated 6/1/15, does not include the time the parent was notified of the child's injury. 2. The record of an injury to child 4, dated 4/21/15, does not include the date and time the parent was notified of the injury.

Plan of Correction: A training will be conducted during our staff meeting Friday, July 31, to discuss complete documentation of children's injuries. A new injury record may be developed.

Standard #: 22VAC40-185-570-E
Description: Based upon observation, the facility has not ensured that prepared infant formula is dated and labeled with the child's name. Evidence: Upon arrival of the inspector into the infant room at 1:40 PM, child 1 was in a bouncer seat holding a prepared bottle. The bottle was not labeled with the name and date. Staff stated that the label with the name and date must have come off when the bottle was placed in the bottle warmer.

Plan of Correction: Staff will be re-trained at our staff meeting on Friday, July 31, to ensure that when a prepared bottle is given to an infant, the bottle is clearly labeled with the name and date.
