22VAC40-191 Background Checks (22VAC40-191)

An "other" inspection was conducted by the licensing inspector in response to a self reported incident. A monitoring inspection was conducted following the "other" inspection. All required information was posted and a sample of staff and children's records were reviewed today. Some but not all standards were checked today.

The "other" inspection concerned a child left on the playground for approximately 1 minute (prior to name to face attendance being taken in the classroom). See violation page for additional information.

A monitoring inspection was conducted today. Observations were done in all classrooms. Children were having lunch or transitioning to lunch. Infants had already eaten and were either sleeping, on the floor, or sitting in infant equipment.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the facility did not ensure the required minimum depth of resilient surfacing a fall zone. Evidence: There is a red slide and a green slide in the K-1st grade playground. The end of the exit chute of both slides touches the ground. There are areas in the fall zones where the depth of the gravel is less than 6 inches deep and in several areas the ground is visible. There are also several areas where the ground is visible and other areas where there is more than 6 inches of gravel in the fall zones of the nearby swings.

Plan of Correction: Maintenance will be contacted and gravel will be moved/replenished as needed to ensure the 6 inches. The equipment will not be used by the children until this has been corrected.

Standard #: 22VAC40-185-340-F
Description: Based on information provided through interviews and documentation, the center did ensure sight and sound supervision of a child under 10 years of age. Evidence: On 5/19/2015, three staff and 12 toddlers were outside on the playground for approximately 30 minutes. They went back into the classroom, closed the door, did a name to face head count and realized a child had been left on the playground. At the same time, a staff person the playground knocked on the door returning the child to the classroom. The center requires staff to do a head count in the classroom before going outside, and to do a head count before returning to the classroom.

Plan of Correction: Staff were retrained regarding the head count procedure. A redesigned name to face head count form is now being used to ensure all children are accounted for.
