Technical Assistance:
Consultation today included first aid supplies (icepacks) and electrical outlets.

Comments:
An unannounced renewal inspection took place today from 11 am - 1 pm. There were 32 children present with six staff. I observed children leaving the park and walking back to the center; transition to morning activities; lunch and the beginning of naptime. The preschool area has been renovated with child-accessible learning centers with an emphasis on preparation for kindergarten. Standard #: 22VAC40-185-60-A
Description: A review of children's records today found that some required information was missing. Evidence: 1. Of three children's files reviewed, Child # 1 did not have complete parent employment information or complete emergency contact information. 2. None of the files reviewed today had documentation of confirmation of up-to-date information by the parent.

Plan of Correction: Update Child#1's Child Info Form 7/30/15 We will have all parents review their file, make any required changes, and sign and date a page indicating they have done this. 8/7/15. A form has been prepared which shall be used each time we release our bi-annual report cards. This form shall be signed and dated, indicating whether the parent has accepted or rejected the opportunity for a parent teacher conference. 7/30/15

Description: During today's inspection, a review of records found that there was no documentation of scheduled opportunities for parent teacher conferences.
