Comments:
An unannounced monitoring inspection was conducted in the Center on July 31, 2015. Upon arrival, the LI observed 24 children in care. The children were in two classrooms, one group was supervised by two staff members and one group was supervised by three staff members. As reported 36 children are currently enrolled, a total of 29 children had attended on the date of inspection. The LI observed a group of children during outdoor play. The children were served an afternoon snack. A group of children in classroom A. were observed as they participated in a circle activity identifying numbers 1 to 10. Staff were engaged with the children and participated in their activities. A sample of children's records was reviewed. Documentation of practiced emergency drills was reviewed, first aid kits were supplied with the required items. Medications and written authorizations to administer were reviewed for 2 children. A discussion was held with the Center Lead Teacher regarding playground equipment. One of 6 swings had visible rust on the chains. A maintenance request was previously submitted. As reported, the swing will be removed from the equipment until chains are replaced. The inspection was conducted from 3:15 to 5:30PM. If you have any questions, please call (540) 309-2310. Standard #: 22VAC40-185-140-A
Description: Based on record review, the Center failed to obtain documentation of a completed physical examination for a child whom has been enrolled in the Center over 30 days. As cited in the violation report dated April 28, 2015 a child's record (child #3, enrolled 02/23/2015) did not contain documentation of a physical examination by or under the direction of a physician.

Plan of Correction: Notification will be given to the child's parent by the responsible staff member. Documentation of a completed physical examination will be obtained and filed in the child's record.
