63.2 Facilities and Programs..
An unannounced inspection was conducted today regarding supervision of children based on a report made by the center. There was a total of 65 children, supervised by 10 staff. The children were preparing to leave for a field trip a National's game. The physical plant and records were reviewed and staff interviews were conducted. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-340-F Description: Based on staff interviews, a six year old child who was left alone in the classroom bathroom was not always within sight and sound supervision of staff. Evidence: 1. On July 21, 2015 at approximately 4:00pm, a six year old child was left in the bathroom located inside one of the classrooms for approximately two minutes. Staff interviews confirmed that the children were transitioning from the cafeteria back to the classroom. Upon arrival to the class, Child 1 stated that she needed to use the bathroom. Staff A informed Staff B that Child 1 was in the bathroom. 2. The class left to walk back to the cafeteria and Child 1 was left behind. Child 1 exited the bathroom crying when noticed by a parent. At this time, a Center Supervisor (Staff D) was in the classroom next door and heard the child and the parent. Staff D then went over to the other classroom. Plan of Correction: Not available online.
