An unannounced monitoring inspection was conducted today. There were 109 campers in attendance, with a total of 17 staff members assigned to their direct supervision. The campers were observed enjoying a large variety of activities during this inspection, including: making tie-dye shirts, decorating purses/wallets, rehearsing their weekly theater production, painting scenery for the theater production, playing indoor flag football, playing a ball toss game, and painting string bags. This inspection was completed between approximately 10:00 am and 11:50 am. Thank you for meeting with me today. Standard #: 22VAC40-185-60-A Description: Based on a review of children's records, it was determined that three records were not complete. Evidence: 1. Child record #1 does not include the phone number for one emergency contact person. 2. Child record #2 does not include a physical exam and shot record. 3. Child record #5 does not include each parent's work phone number. Plan of Correction: The parents will be reminded to supply the missing information.
Standard #: 22VAC40-185-280-B Description: Based on observations, it was determined that two potentially hazardous substances were not locked. Evidence: In the Performers 1 classroom, two bottles of air freshener were stored in an unlocked lower level cabinet beneath the sink. Plan of Correction: The air fresheners were moved during the inspection.
Standard #: 22VAC40-185-510-C Description: Based on a review of children's medications, it was determined that one medication was expired. Evidence: One child's long-term prescription medication expired on 7/31/15. Plan of Correction: The child no longer attends this camp and will not be attending this school in the Fall. I will contact the parent to come pick up the medication.
Standard #: 22VAC40-185-520-B Description: Based on observations, it was determined that the children's sunscreen bottles were not stored in a manner to keep them inaccessible to the children. Evidence: Upon the Inspector's arrival, the bin of children's sunscreen bottles was stored on the floor in the hallway where children walk regularly throughout the day. Plan of Correction: The bin of sunscreen was moved to the office during the inspection.
