Comments:
An unannounced monitoring inspection was conducted beginning at 2:35 p.m. and ending at 5:25 p.m. with the director. There were sixty-three children in care with ten staff providing supervision. Infants were observed having playtime on the floor and sleeping. An older infant was observed during diapering. Bottles were labeled and dated. Toddlers, two year olds and three year olds were observed having snack which consisted of crackers and cheese and applesauce. Four year olds were observed during story time. Older school aged children were observed lining up for outside time. Younger school age children were observed finishing snack and lining up for outside time. The playground was observed. Five children's records and three staff records were reviewed. Please forward the plan of correction/date to be corrected by for each violation by 8/7/2015 at noon. Description: Based on review of documentation, not all children's records were complete. Evidence: Child #2, #3 and #4 did not have complete addresses for a second emergency contact.

Description: Based on observation, not all areas of the center were maintained in a safe, operable condition. Evidence: In the older school age classroom (SAC B), there is a hole in the drywall near the floor. There is a hole in the door to bathroom.

Standard #: 22VAC40-185-330-B
Description: Based on observation, it was determined that the mulch in fall zones on the playground was not resilient. Evidence: The mulch in the fall zones of the climbing wall and climbing rope wall was not resilient. The mulch was decomposed exposing dirt in the fall zone areas. Resilient surfacing is required to be at least six inches in depth, six feet on all sides of the stationary equipment.
