22VAC40-191 Background Checks (22VAC40-191)
The licensing inspector conducted an unannounced monitoring inspection from 1:45pm - 4:30pm. There were thirty three girls in care with eight staff. The children were observed arriving at the center, at free play, playing on the playground and in group activities. Four staff and four children's records were reviewed. Standard #: 22VAC40-185-60-A-7 Description: Based on record reviews, the center failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed four children's records. One of four records reviewed (child record #3) listed allergies to ants, bees, wasps, and spiders; however, actions to take in an emergency situation were not listed. Plan of Correction: The director of operations contacted the child's parents today and requested this information. The children's records will be reviewed to ensure all information is complete.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center failed to maintain the playground in a safe condition. Evidence: 1. The inspector observed the playground. There is a broken landscape timber surrounding the bark mulch that has sharp edges. Plan of Correction: The Chief Operating Officer (COO) said the landscape timber has been placed on a list of items that need repaired and it will be replaced. Staff will monitor the playground for needed maintenance.
Standard #: 22VAC40-185-540-C Description: Based on observation, the center failed to have all required items in a van's first aid kit. Evidence: 1. The inspector inspected the vans and observed the first aid kits. The second van that was inspected had a first aid kit that did not contain band aids, scissors or tweezers. Plan of Correction: These items were placed in the first aid kit during the inspection. Staff will monitor the first aid kits to ensure they contain all required supplies.
Standard #: 22VAC40-185-550-H Description: Based on observation, the center failed to have the emergency document on one of the vans. Evidence: 1. The inspector inspected the vans. The first van inspected did not contain the emergency document containing emergency contact information, potential shelters, hospitals, evacuation routes, etc. Plan of Correction: The COO will place this document on this van by tomorrow. Staff will monitor the vans to ensure this document remains on the van.
