Comments:
An investigation was conducted today in response to a self-reported incident received by our office regarding supervision of a two year old. Based upon documentation reviewed, and multiple staff interviews, the preponderance of evidence supports violations. Other violations not related to the self-reported incident were also cited. Standard #: 22VAC40-185-160-A
Description: Based upon documentation reviewed, a staff member did not submit documentation of a negative tuberculosis (TB) screening no later than 21 days after employment. Evidence: One of the three staff records reviewed did not contain a tuberculosis (TB) screening on file (Staff C; DOH 06/11/2015).

Standard #: 22VAC40-185-70-A
Description: Based upon documentation reviewed, staff records were incomplete. Evidence: 1) One of the three staff records reviewed did not contain the name, address and telephone number of a person to be notified in an emergency (Staff C). 2) Two of the three staff records reviewed did not contain written information to demonstrate the staff completed the licensing orientation training by the end of the first day of assuming job responsibilities (Staff C; DOH 06/11/2015 and Staff D; Date of hire (DOH) 06/23/2015).

Description: Based upon observations made, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: Two bottles of multi-surface cleaner and one sanitizer spray bottle were inside an unlocked cabinet in the Infant B classroom.

Standard #: 22VAC40-185-340-F
Description: Based upon documentation reviewed and multiple staff interviews, a child was not kept within actual sight and sound supervision of a staff. Evidence: On Wednesday July 15, 2015, Staff A was changing 6 children (2 years old) in the Preschool A classroom. After diapering was done, Staff A lined up the children in the Preschool B classroom near the back door then went outside to the playground. At approximately 5:18pm, a parent went to get his/her child's belongings from the Preschool B classroom and found Child A (2 year old) alone and unsupervised in the classroom. The parent found a staff member who was standing nearby (Staff B) and Staff B spoke to the child and brought her back to Staff A. Staff interviews confirmed that Child A was not within actual sight and sound supervision of a center staff for approximately 3-5 minutes. Additionally, documentation reviewed confirmed that after checking all children on the Child Supervision Record (CSR), Staff A noticed Child A, "was not on the playground went back to Preschool B classroom a few minutes later at [approximately] 5:18pm". [sic]

Description: Based upon observations made and interviews, a separate leakproof storage system was not used when cloth diapers were used for a toddler in the Infant A classroom. Evidence: The Licensing Inspector (LI) observed diapering procedures during today's inspection in the Infant A classroom. After Child C's soiled cloth diaper was changed, Staff E placed the cloth diaper inside a plastic bag and stored the plastic bag inside the child's cubbies in the Infant B classroom. Staff interviews confirmed that a leakproof covered receptacle was not used for wet or soiled cloth diapers.
