Comments:
An unannounced renewal inspection was conducted from 8:30 am-11:15. At the time of entrance, 55 children were in care with 15 staff members present. Children were observed during snack time, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review, licensee has not maintained a complete record for each staff. Evidence: Staff #1, hired 8/2014 and staff #2, hired 2/2013 does not have documentation of at least two references on file.

Plan of Correction: References/copies are being located and will be in the files within a week.

Standard #: 22VAC40-185-270-A
Description: Based on observation, licensee has not maintained all areas and equipment in an operable condition. Evidence: In room #143 the door handle on the cabinet above the sink is broken.

Plan of Correction: Temporary child safety lock is on cabinet. Request for repair of cabinet has been made.

Standard #: 22VAC40-185-510-N
Description: Based on a review of the medications, licensee has not notified the parent that the authorization to administer medication form has expired. Evidence: Child #1 medication authorization form for Proair inhaler expired on 7/11/15.

Plan of Correction: Medications and forms removed to return to parent. Parent called.
