Comments:
An unannounced monitoring inspection was conducted today from 11:05am-12:10pm. There were 21 preschool children directly supervised by 3 staff. The physical plant, 3 staff records, 3 children?s records, medications, evacuation drills, injury reports, and emergency supplies were inspected. Children were observed working independently by completing puzzles, reading, and playing with blocks. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review, one of three staff records reviewed was incomplete. Evidence: Written information to demonstrate that the individual possesses the orientation training required by the job position was not observed documented for Staff C.

Plan of Correction: Review orientation form with staff member and put form in staff file.

Standard #: 22VAC40-185-510-C
Description: Based on medication review, the center did not follow their medication policies. Evidence: A medication for Child 4 was observed on site with an expiration date of June 2015.

Plan of Correction: Ask parent to provider new medication.

Standard #: 22VAC40-185-550-D
Description: Based on record review, the center did not implement monthly practice evacuation drills. Evidence: A practice evacuation drill was not observed documented for the months of June 2015 and July 2015.

Plan of Correction: Hold additional drills in August 2015 and September 2015. Document on official record
