Thank you for your time and assistance regarding the unannounced inspection that was conducted on August 14, 2015. The on-site visit was from approximately 11:30 a.m. to 1:25 p.m. The inspection involved an incident that was self-reported to the Valley Licensing Office on August 13, 2015. Two staff members were interviewed during the visit. One staff record was reviewed, one children's record was reviewed, and one classroom was observed. The exceptions to compliance were noted on the violation notice and the risk ratings associated with the violations were noted on the supplemental page. There were 72 children present and in the care of 13 staff members with additional staff members available to assist if necessary. There were a total of 131 children enrolled in the program. Please complete the ?plan of correction? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Standard #: 22VAC40-185-240-A Description: Based on review of staff records and discussion with staff, it was determined that center failed to provide one staff member with the orientation training required by the end of their first day of assuming job responsibilities and the written information required to give a staff member by the end of their first day supervising children. Evidence: 1. Staff # 2 had a hire date of 06/16/2014. There was no documentation in her file stating that she had received the required orientation training. 2. Staff # 2 could not verbally confirm that she had received orientation training on her first day of employment. Plan of Correction: The director will ensure that all new employees will receive orientation by the end of their first day supervising children effective 8/14/2015.
Standard #: 22VAC40-185-340-A Description: Based on staff interviews, a parent interview, review of supporting documentation, and verification from a staff member, it was determined the center failed to ensure the care and protection of one child when he left the center to meet his parent in the parking lot. Evidence: 1. On 08/12/2015 at approximately 5:00 p.m., Staff #2 reported that Child #1 (7 years old) saw his mother drive into the parking lot of the center through the window and he immediately stopped what he was doing and went out into the main hallway of the center by the front entrance. Staff #2 stated that she did not go after the child and she did not know where the child went after he entered the hallway. 2. The mother of Child #1 stated that she parked her car in front of the center and as she was getting out of the car Child #1 was approaching the car and he was not in the care of a staff member. She then took her child back into the center and reported the incident to the director. Plan of Correction: The staff member who failed to ensure the care and protection of the child was immediately terminated. The director retrained all staff on the licensing standards related to this violation on 8/14/2015.
Standard #: 22VAC40-185-340-D Description: Based on review of one staff member's record and verification from staff, it was determined that there was not a staff member who met the qualifications of a program leader present in one grouping of children. Evidence: 1. During the 08/14/2015 inspection, the qualifications of Staff #2 were reviewed and there was no documentation in her file which demonstrated that she met the qualifications of a program leader. 2. Staff #1 and Staff #2 both stated that on 08/12/2015, Staff # 2 was the only staff member present during the day with the children in Classroom #1. Plan of Correction: Effective 8/14/2015 the director will ensure that a program leader is present with each grouping of children as stated in the standards.
