22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 11:00am-1:00pm. Upon arrival there were 68 children in care with 14 staff present. Children were observed eating lunch, during a bathroom break and nap. Babies were observed eating in high chairs and playing on the floor. A sample of 2 staff and 4 children's records were reviewed in addition to 2 over-the-counter skin products and 4 medications.
Standard #: 22VAC40-185-270-A Description: Based on observation the center failed to ensure areas and equipment of the center, inside and outside were maintained in a safe and operrable condition. Evidence: 1. On the playground the fence dividing the two play areas had a broken hinge. When the door opened it fell over and was hanging by only the bottom hinge. 2. On the playground the wooden perimeter fence had 2 broken boards in reach of children that measured about 8" and had jagged splintered edges. 3. In the 4 year-old room there was an opened box of plastic gloves on a cubby used for a learning center used by the children. 4. In the infant room there was an art display showing an activity the babies engaged in on 08/12/15. The activity used small hard and uncooked spiral macaroni and packing peanuts in a sensory table. The materials were displayed on the wall along with pictures of the babies playing with the materials. Both materials fit inside of a choking tube and were the size and consistency that may pose a choking hazard to children under 2 years. Plan of Correction: 1. Gate was repaired on 8/25/15. Management will ensure daily inspections of the playground. 2. This is in process of being repaired prior to inspection visit. This will be fixed by 9/15/15. 3. Gloves were moved to a safe storage area the day of the visit. Teachers were retrained and management will ensure compliance through periodic inspection. 4. Staff will be re-trained on appropriate materials to use in the younger classrooms. Choke tubes are in each classroom. Management will ensure compliance through periodic inspection.
Standard #: 22VAC40-185-280-B Description: Based on observation the center failed to ensure hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: In the toddler room the two staff in the room were standing in the opened storage closet area and the children where in the middle of the room in front of the closet. The door underneath the changing table was wide open and had a bottle of disinfectant spray inside. Plan of Correction: Staff members were retrained on proper procedures regarding hazardous materials. This was corrected at time of visit. Management will ensure compliance through periodic inspection.
Standard #: 22VAC40-185-330-B Description: Based on observation the center failed to ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-9. Evidence: On the playground one of the composite structures there was 1" of mulch in the fall zone of the slide and tarp was exposed. The other composite structure is under a turf-like surfacing. In the fall zone of the slide the turf has separated and much was filled in this space, mixing the two surfaces. Plan of Correction: Management will fluff mulch at the bottom of the slide on 8/21/15 and will ensure that the levels of mulch meet standards through periodic inspections. All work will be completed by 9/21/15.
