Comments:
The licensing inspector conducted an unannounced renewal inspection August 25, 2015 from approximately 10:10am to 1:40pm. The following ratios were observed with a total of 40 children present in the Preschool building during the inspection: Infants - 14 children with 4 staff Ones ? 5 children with 1 staff Twos - 14 children with 2 staff Threes & Fours - 5 children with 1 staff School Agers - 2 children with 1 staff The children were engaged in a variety of activities when the licensing inspector arrived. Infants were observed playing in the floor with their teachers, sleeping in their cribs, listening to music and sitting in swings. The Ones were observed with toys and eating lunch. The Twos, Threes and Fours classrooms were observed playing outside, eating lunch, working puzzles, playing with toys and eating lunch. The School Age children were observed completing worksheets. Lunch consisted of Salisbury steak and fish patties, green beans, fruit and milk. All areas of the center including classrooms, hallways, kitchens, playgrounds, and bathrooms were inspected. The center was equipped with toys and supplies and items were available to the children. The following information was reviewed: updated injury prevention plan, injury reports, daily sheets, daily attendance, emergency preparedness plan, practiced emergency drill logs children?s assessments, parent agreements, staff training information and liability insurance. The center's first aid kit and emergency supplies were inspected. The owner stated medication is not administered at this time. The center provides transportation and vehicles were inspected. Last documented health inspection: 03/26/15 Last documented fire inspection: 02/24/15 During the inspection, five children records and four employee records were reviewed. The risk profile tool will be used for the licensing recommendation once all of the necessary paperwork is received by the licensing department. The renewal application and required paperwork will need to be submitted prior to the license expiring on September 13, 2015. Standard #: 22VAC40-185-520-C
Description: Based on observation, the center did not ensure all diaper ointments were labeled with the child's name. Evidence: During the inspection, the licensing inspector observed a diaper ointment located in the Infant room cabinet. The diaper ointment was not labeled with the child's name.

Plan of Correction: Per owner - We will review with all teachers the requirements of checking and labeling all diaper ointments.
