Comments:
An unannounced renewal inspection was conducted on 8/20/15 between 1:00 and 6:00. The inspection was conducted with the director. The inspector spoke with staff and children in all classrooms. The following staff/child ratios were observed: Infants ? two staff with seven children (two classrooms) Toddlers - one staff with four children 2?s - two staff with 15 children 3?s - three staff with 21 children 4 and 5?s - two staff with 20 children 5?s ? one with eight children School age - two staff with 21 children (two classrooms) The children were engaged in various activities including playing games, making dream catchers, reading, and playing on the playground. Snack this afternoon was tater tots and milk. Lunch today was tuna sandwich, carrots, pineapple and milk. Health inspection ? 4/29/15; Fire inspection ? 4/3/15; Last fire drill 7/15/15 and two shelter-in-place drills were conducted All required documents were posted including license, licensing notices, menu, and evacuation information.

Violations:
Standard #: 22VAC40-185-60-A
Description: A review of five children's records found that the center had failed to obtain the full address for one child's emergency contact. Evidence: Of five children's records reviewed, one child, Child #3, was missing the house number for one of the listed emergency contacts. The staff could not find an address for the contact.

Plan of Correction: Request info from parent and make sure info is correct going forward by having director sign of on folder.

Standard #: 22VAC40-191-40-D-1-C
Description: A review of three staff, one new and two already employed, found that the center failed to obtain updated State Police Criminal Record Checks (CRC's) for the two employees before three years had passed from the date of the previous CRC. Evidence: Records reviewed for Staff #2 found that the last CRC was dated 5/16/11. An updated CRC was required by 5/15/14. Records reviewed for Staff #3 found that the last CRC was dated 5/17/11. An updated CRC was required by 5/16/14. No updated CRC's were found for the two staff. The other background checks (Sworn Statement and Child Protective Services (CPS) were filed in the records.

Plan of Correction: Request being done today and spreadsheet created and maintained to make sure all checks are done timely.
