Comments:
23 Children were in care during the inspection. The age range of the children was seven weeks to four years old. Children were observed during rest time,free play. Four staff a current in MAT. Two staff and six children's records were reviewed. Inspection time 12:30pm-3:00pm

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of children's records not all were complete. Evidence: Child A with a start date of 11/19/13 was missing a work number for father and the physician's phone number. Child B with a start date of 6/30/13 was missing work number for father .

Plan of Correction: I will ask the parents for the missing information.

Standard #: 22VAC40-185-510-D
Description: Based on review not all medication authorization's were complete. Evidence: Child C's medication did not match what was written on the medication authorization.

Plan of Correction: We will have the parent update the medical authorization form.
