Technical Assistance:
1. Reviewed child enrollment requirements and discussed electronic records vs. paper documents, to include what is needed for children receiving back-up care and attending at more than one Bright Horizons center. 2. Discussed body language of staff when implementing behavior guidance. 3. Discussed potential playground renovations. 4. Documents should be available for review during the inspection vs. submitting them post-inspection.

Comments:
Conducted an unannounced monitoring inspection. Observed 46 children + 12 direct-care staff. Ratios and supervision were in compliance. Children were observed having outdoor water play and play time on the playground, reading books, eating lunch, and taking naps. The infants were napping and being held. Staff were actively involved with the children and assisting them with tasks in all groups. Lunch served: chicken sausage, pasta, peas, bread, oranges, and milk. The center was found to be clean, safeguarded appropriately, and sufficiently supplied with toys and equipment for the children. Medication management was in compliance. Required documents were posted. Record keeping violations found. Inspection completed 11-25am-1:30pm. Questions about this inspection may be directed to pamela.sneed@dss.virginia.gov

Description: Based on 3 children's records reviewed, it was determined that 1 record did not include documentation of immunizations. Evidence: Child #3 - First day of attendance is today, 8/28/15.

Plan of Correction: Requested parent's Child's immunization [sic]

Standard #: 22VAC40-185-240-A
Description: Based on 4 staff records reviewed, it was determined that 1 record did not include documentation of orientation training. Evidence: Staff #7 - Date of hire was in March 2015.

Plan of Correction: Completed

Standard #: 22VAC40-185-440-J
Description: Based on observations made, it was determined that there was not a minimum of 30" clearance on the service side of occupied cribs. Evidence: 2 occupied cribs were sharing a clearance space of approximately 16".
