Comments:
Thank you for your assistance during this unannounced renewal inspection. This on-site visit was conducted on September 3, 2015 from 11:15 a.m. through 1:50 p.m. On this date, there were one-hundred thirty-four (134) children in the care of twenty-two (22) staff members in nine classrooms. There were five (5) children's records and ten (10) staff records reviewed and updated. The medication and medication paperwork was reviewed for three (3) children. Areas of non-compliance may be found on the violation notice. Areas observed were educational center activities, story time, music, hand washing/bathroom and diapering procedures, transition, outside play, lunch, infant feeding, nap and departure. Lunch was observed to meet USDA guidelines and consisted of bologna and cheese sandwich, pineapple, salad, and milk. All interactions between staff and the children were appropriate and the children seemed comfortable with the daily routine. Children have been completing seasonal activities and artwork is displayed throughout the center. There are a wide variety of age/stage materials for the children in care. Standard #: 22VAC40-185-350-E-4
Description: Classroom # 8 was out of ratio for approximately one minute. Evidence: Based on observation only one staff was in the classroom with fourteen children, ages three to four years, for approximately one minute.

Plan of Correction: Staff will be reminded if they need to leave the classroom for any reason that would put the classroom out of ratio, they will contact the office to send a staff member to cover and maintain required ratios.

Standard #: 22VAC40-185-370
Description: Two infants in Classroom # 1 did not have documentation of receiving "tummy time" for September 3, 2015. Evidence: Based on record review and conversation with the staff it was determined "tummy time" was not documented for the two infants in care.

Plan of Correction: Staff will be reminded to document "tummy time" daily for all infants.
