Comments:
Thank you for your assistance during this unannounced monitoring inspection. The on-site visit was conducted on August 31, 2015 from 11:00 a.m. through 1:00 p.m. There were thirty-four (34) children in the care of nine (9) staff in five classrooms. There are a total of seventy-four (74) children enrolled. Four (4) children's record and two staff records were reviewed and updated. There is no medication being administered at this time. Areas of non-compliance may be found on the violation notice. Areas observed were educational centers, circle time, hand washing/bathroom and diapering procedures, lunch, transition and free choice activity and nap. Lunch was observed to meet the USDA guidelines and consisted of fish nuggets, peaches, bread and milk. All interactions between staff and the children were appropriate and the children seemed comfortable with the daily routine. Violations:
Standard #: 22VAC40-185-130-B
Description: Child A did not have documentation of receiving immunizations within the last six months. Last documented update was 12/12/2014. Documentation of child's birth date is 08/29/2014. Evidence: Based on record review it was determined Child A did not have documentation of obtaining updated immunizations within the last six months.

Plan of Correction: The director will request the parent provide documentation of updated immunizations. In the future, children's records will be reviewed every six months to ensure all immunization documentation is up to date.

Standard #: 22VAC40-185-160-A
Description: Staff A did not have documentation of obtaining a tuberculosis screening within 21 days of hire. Evidence: Based on record review it was determined Staff A did not have documentation of obtaining a tuberculosis screening within 21 days of employment.

Plan of Correction: The director will request the Staff A obtain a tuberculosis screening and provide documentation.

Standard #: 22VAC40-185-60-A
Description: Child B did not have written documentation of the first day of attendance. Evidence: Based on record review it was determined Child B did not have documentation of the first day of attendance.

Plan of Correction: The first day of attendance will be documented for Child B. In the future, the first day of attendance will be documented when the students begins care.

Standard #: 22VAC40-185-330-B
Description: Resilient surfacing on the outside play structure slide measured less than three (3) inches. Evidence: Based on measurement it was determined the mulch, at the bottom of the outside slide, measured less than three (3) inches in depth.

Plan of Correction: United Way volunteers will be adding and spreading mulch next Tuesday, September 8, 2015. Mulch will checked weekly and raked. New mulch will be added if measurements are not at least six (6) inches in the required fall zones.
