22VAC40-191 Background Checks (22VAC40-191)
Arrived 11:30am Departed 3: 20 pm An unannounced monitoring inspection was conducted today. Eleven infants and toddlers were present with three staff. Nine toddlers and twos were present with two staff. Twenty-seven preschoolers were present with four staff members. Sixteen school-age children were present with one staff member. The children were reading books, having group time, playing outdoors, eating lunch and resting. Five children's files and nine staff files were reviewed. Thank you for your cooperation. Standard #: 22VAC40-185-160-A Description: Based on the review of nine staff records, the facility did not maintain complete records for all staff. Evidence: 1. There are no TB test or screening results on file for staff # 1,2,3,6 & 8. All staff have been employed longer than 21 days. Plan of Correction: All risk assessments will be complete and copies e-mailed to licensing representative.
Standard #: 22VAC40-185-270-A Description: Based on a tour of the playground, the facility did not maintain all equipment in a safe condition. Evidence: 1. On the playground, at the base of a fence post, the top of a concrete footer is exposed. Plan of Correction: A planting area will be constructed to fill in the area with dirt, mulch and flowers.
Standard #: 22VAC40-185-440-J Description: Based on a tour of the infant room, the facility did not maintain the required clearance around each occupied crib. Evidence: 1. Crib #6 was occupied during the inspection and the clearance around the crib was 15 inches. Plan of Correction: Furniture/cribs will be moved to correct clearance issues.
Standard #: 22VAC40-185-510-C Description: Based on a review of the medication for children in care, the facility did not obtain all required written permission. Evidence: 1. The file for child #2 does not contain written long-term permission, from a physician, for an inhaled medicine. 2. The permission form in the file is dated 8/6/15 and has expired. Plan of Correction: A new form will be completed by the parent and a new form will be sent to the physician.
Standard #: 22VAC40-185-570-E Description: Based on a tour of the infant room, the facility did not maintain label each infants' bottle. Evidence: 1. The LI observed three infant bottles in the refrigerator. 2. They were not labeled with the child's name and date. Plan of Correction: Bottles will be labeled with child's name and date. Nursery supervisor will check these each day.
Standard #: 22VAC40-191-60-C-1 Description: Based on the review of nine staff records, the facility did not maintain complete records for all staff. Evidence: 1. There is no criminal background check on file for staff #9. (hire date 6/8/15) Plan of Correction: New forms will be resubmitted. The staff member will not be scheduled to work until we have received cleared background check.
Standard #: 22VAC40-191-60-C-2 Description: Based on the review of nine staff records, the facility did not maintain complete records for all staff. Evidence: 1. There is no CPS Clearance on file for staff #9. (hire date 6/8/15) Plan of Correction: New forms will be resubmitted. The staff member will not be scheduled to work until we have received cleared background check.
