Technical Assistance:
There shall be documentation on site to demonstrate that each employee's Criminal History Record Check and Child Protective Services Registry check requests were submitted within the required time frame.

Comments:
An unannounced monitoring inspection was conducted today. There were 38 children in care, with a total of 4 staff members assigned to their direct supervision. The children were observed assembling puzzles, creating art projects, playing with creative play toys, and eating snack brought from home. This inspection was completed between approximately 9:00 am and 10:15 am. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-185-240-A
Description: Based on a review of new staff training documentation, it was determined that there is no documentation available to show that staff person A completed all required components of First Day Orientation Training. Evidence: There is no documentation to show that staff person A completed "Recognizing Child Abuse and Neglect" training.

Plan of Correction: She completed the training. I will place the documentation in her file.

Standard #: 22VAC40-185-280-B
Description: Based on observations, it was determined that various potentially hazardous substances were not stored in a locked place. Evidence: In Preschool room A, three bottle of spray paint were stored in an unlocked, upper level cabinet. In addition, several containers of sanitizing table wipes were stored on top of a row of upper level cabinets.

Plan of Correction: All of the table wipes and the bottles of spray paint were up high and completely out of the reach of the children. All of these items will be moved to a locked cabinet.

Standard #: 22VAC40-185-330-B
Description: Based on observations, it was determined that there was not enough resilient surfacing in place in many of the playground fall zones. Evidence: There was not enough mulch at the bottom of the slides, beneath the climbing ladder, and surrounding the stepping posts. In each of these areas the mulch has worn down to dirt.

Plan of Correction: The maintenance department has already scheduled to put more mulch on the playground. I will follow-up to make sure the mulch is put in place this week.
