Comments:
An unannounced monitoring inspection was conducted on September 16, 2015 from approximately 10:30 am to 1:00 pm . Five staff were present with nineteen children. Additional staff were on site for administrative and kitchen duties. Children were observed during circle time, music and free play. Three children?s records and two staff records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, diapering procedures, hand washing procedures, nutrition, special feedings, injury report documentation, medication, children and staff records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. Violations were reviewed with and verified by the Administrator on this date.

Standard #: 22VAC40-185-270-A
Description: Based on observation, facility did not ensure areas and equipment of the center both inside and outside are maintained in a clean, safe and operable manner. Evidence: On September 16, 2015, the following was observed: 1. The sinks in the preschool classroom and children's front bathroom are beginning to separate from the wall causing the sink to not be securely attached. 2. The carpet at the preschool doorway is frayed with the floor matting curling up causing a potential tripping hazard. In the same area, the carpet is buckled presenting a potential tripping hazard. 3. A laminate covering is separating from the wall outside of the girls' bathroom leaving potential pinch points. 4. A radio is stored on a shelf in the toddler/2's classroom. The cord is hanging within reach of children and could be pulled off the shelf. 5. The shed on the side of the outside playground is warped leaving rusted sharp edges. 6. A wooden crate is propped against the shed on the playground which has splintered edges. 7. The exterior of the door by the shed is peeling. 8. There are 7 nails protruding from the wood fence frame by the picnic tables on the playground.

Plan of Correction: 1. Maintenance man will come to make repairs this week. 2. Will get carpet replaced. 3. Maintenance man will come to make repairs this week. 4. Radio will be taken out of room today. 5. Replaced within 30 days. 6. Will take it out front for trash today. 7. Will be replaced within 30 days. 8. Maintenance man will come to make repairs this week.

Description: Based on observation, facility did not ensure hazardous chemicals are kept locked. Evidence: On September 16, 2015, Clorox disinfectant wipes labeled "Caution...Keep out of Reach of Children" was observed stored on a shelf in the children's front bathroom.

Plan of Correction: Wipes will be locked up.

Description: Based on observation and interview, facility did not ensure required nonmedical emergency supplies are available. Evidence: On September 16, 2015, there were not batteries available to operate the radio.

Plan of Correction: Administrator will get some today.
