22VAC40-191 Background Checks (22VAC40-191)
Today's visit was routine monitoring inspection. The inspection began at 10:00am and concluded at 2:00pm. There were thirty nine children present with six direct care staff and two additional staff members. During the inspection picture day was observed, all three classrooms were observed, morning snack time was observed, emergency supplies were viewed, nap time was observed, four out of twelve staff records were reviewed, four out of fifty five children's records were reviewed, the playground was viewed, a sample of five injury reports were viewed and medication was present at the center and was viewed along with authorizations. Standard #: 22VAC40-185-160-C Description: Based on the inspector's review of four out twelve staff records, the center failed to ensure a staff member had an up to date TB screening every two years as required by the standard. Evidence: The record for staff member #1 has a TB screening dated 8/5/13 and is more than two years old. Plan of Correction: The staff member will complete a TB screening within one week of today's inspection. Additional staff records will be audited to ensure TB screenings are up to date.
Standard #: 22VAC40-185-280-G Description: Based on the inspector's observations, the center failed to ensure a substitute container with a hazardous substance was clearly labeled. Evidence: The inspector observed two janitorial spay bottles labeled "Floor" in locked cabinets of two separate classrooms. It was not clear as to what the spay bottles contained until a staff interview took place and a staff member stated it was a floor cleaner. Plan of Correction: Both bottles will be labeled with the actual cleaner name. Staff will be reminded to keep all generic containers clearly labeled with the contents.
Standard #: 22VAC40-191-40-D-1-B Description: Based on the inspector's review of four out of twelve staff records, the center failed to ensure a portion of the records had criminal background checks within thirty days of employment. Evidence #1: The record for staff member #3 with a start date of 7/23/15 does not have the results of a criminal record check within thirty days of employment as required by the standard. Evidence #2: The records for staff member #4 with a start date of 7/22/15 does not have the results of a criminal records check as required by the standard. Plan of Correction: The PD will audit records and if the CRC cannot be found a new request will be made within one week of today's inspection.
