Violations:
Standard #: 22VAC40-185-160-C
Description: Based on the inspector's review of four out twelve staff records, the center failed to ensure a staff member had an up to date TB screening every two years as required by the standard. Evidence: The record for staff member #1 has a TB screening dated 8/5/13 and is more than two years old.

Plan of Correction: The staff member will complete a TB screening within one week of today's inspection. Additional staff records will be audited to ensure TB screenings are up to date.

Standard #: 22VAC40-185-280-G
Description: Based on the inspector's observations, the center failed to ensure a substitute container with a hazardous substance was clearly labeled. Evidence: The inspector observed two janitorial spay bottles labeled "Floor" in locked cabinets of two separate classrooms. It was not clear as to what the spay bottles contained until a staff interview took place and a staff member stated it was a floor cleaner.

Plan of Correction: Both bottles will be labeled with the actual cleaner name. Staff will be reminded to keep all generic containers clearly labeled with the contents.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on the inspector's review of four out of twelve staff records, the center failed to ensure a portion of the records had criminal background checks within thirty days of employment. Evidence #1: The record for staff member #3 with a start date of 7/23/15 does not have the results of a criminal record check within thirty days of employment as required by the standard. Evidence #2: The records for staff member #4 with a start date of 7/22/15 does not have the results of a criminal records check as required by the standard.

Plan of Correction: The PD will audit records and if the CRC cannot be found a new request will be made within one week of today's inspection.
