Technical Assistance:
The licensing inspector observed peeling paint on school playground composite.

Comments:
An unannounced monitoring inspection was conducted on September 16, 2015 from approximately 3:35pm -4:10pm . There were 52 children present with four staff members. The children were observed eating snack, homework time and reading activity. Three children's records were reviewed. Exit interview was conducted with site director.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the licensee did not ensure each child enrolled had a record contained with all required information. Evidence: During the inspection conducted on September 16, 2015, record for Child #3 did not have one of the parent's work phone number and child's grade level.

Plan of Correction: Program Director will obtain the needed information from the parent.

Standard #: 22VAC40-185-540-E
Description: Based on observation, the licensee did not ensure there was a working battery operated radio. Evidence: During the inspection conducted on September 16, 2015, the licensee's radio did not have batteries.

Plan of Correction: Admin will purchase batteries for pick up on 9/25.

Standard #: 22VAC40-185-560-K
Description: Based on observation, the licensee did not ensure tables were sanitized before and after snack. During the inspection conducted on September 16, 2015, the children were sitting eating snack at the same tables that were used during reading time. The tables were not sanitized before snack. The licensing inspector verified with the site director that the tables were not sanitized.

Plan of Correction: Program team will ensure that any and all tables used for snack are sanitized before use.
