Comments:
An unannounced monitoring inspection was conducted from 9:00am-11:30am. Upon arrival there were 133 children with 27 staff present. A sample of 5 staff and 5 children's records were reviewed in addition to 4 medications. Children were observed playing games and puzzles in their classrooms and doing worksheets. Class pictures were also observed.

Violations:
Standard #: 22VAC40-185-510-G
Description: Based on observation and a record review the center failed to ensure all requirements were met when administering prescription medication. Evidence: Child #5 had an Epi Pen jr at the center. There were 2 authorization forms, however the one provided by the child's doctor's office stated that an Epi Pen was to be administered.

Plan of Correction: During the inspection administration called the child's doctor's office and confirmed that an Epi Pen Jr was to be administered if needed. Administration will give back the paperwork and medication to the parents and ask for a corrected authorization.
