Standard #: 22VAC40-185-160-A
Description: Based on a review of three staff records, it was determined that the facility did not ensure that each staff member submitted documentation of a negative tuberculosis (TB) screening no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: The TB screening on file for staff #2 (date of employment 8/27/15) was dated 3/13/13 and therefore was completed over 12 months prior to her employment.

Plan of Correction: Staff member will be sent to get an updated TB screening.

Plan of Correction: Staff will have the medication authorization form signed by the parent.

Standard #: 22VAC40-185-510-G
Description: Based on review of the medication, it was determined that the facility did not ensure that prescription medication was in the original container with prescription label attached. Evidence: The albuterol inhaler for child #5 was not in the original container with prescription label attached.

Plan of Correction: Staff will have parent provide the original box.
