Licensing Inspector arrived at 11:45 AM and departed at 1:00 PM. At the onset of the tour, there were 17 children, aged two through school-age, in care with four staff. Children were observed eating lunch, preparing for nap and napping.
Standard #: 22VAC40-185-160-A Description: Based on a review of three staff records, it was determined that the facility did not ensure that each staff member submitted documentation of a negative tuberculosis (TB) screening no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: The TB screening on file for staff #2 (date of employment 8/27/15) was dated 3/13/13 and therefore was completed over 12 months prior to her employment. Plan of Correction: Staff member will be sent to get an updated TB screening.
Standard #: 22VAC40-185-510-A Description: Based on review of the medication, it was determined that the facility did not ensure that prescription medication was given to a child only with written authorizations from the parent. Evidence: The authorization form for the albuterol inhaler for child #5 did not have parental authorization. Plan of Correction: Staff will have the medication authorization form signed by the parent.
Standard #: 22VAC40-185-510-G Description: Based on review of the medication, it was determined that the facility did not ensure that prescription medication was in the original container with prescription label attached. Evidence: The albuterol inhaler for child #5 was not in the original container with prescription label attached. Plan of Correction: Staff will have parent provide the original box.
