An unannounced monitoring inspection was conducted in the Center on September 24, 2015. The LI observed 17 children as they participated in varied program activities. Two staff members were actively supervising the children's activities. Three additional staff members were also present in the Center during part of the inspection. Staff reported 20 children are currently on roll. Activities observed by the LI included: outdoor play, structured circle time as a Teacher read a story to the children, the lunch meal, hand washing procedures, and dismissal at the end of the program day. The Center was equipped with age appropriate materials/equipment for the children's use. Staff were engaged with the children, offering guidance when needed. The LI reviewed a sample of four children's records. Prescribed medications and written authorizations to administer were reviewed for (4) children. Documentation of the Center's annual fire and health inspections was current. The first aid kit and documentation of the practiced emergency drills was in compliance. Standard #: 22VAC40-185-510-E Description: Based on the LI's observations, the Center failed to ensure medications were maintained in the original labeled container, three medications were in foil packages without a pharmacy label. Evidence: The LI observed three medications, for children c. and d., out of the original container, the medications were stored in separate safety saks. The original pharmacy dispensed boxes were flattened and stored with the medications which were in foil packages. Staff put the medications back in the original boxes upon notice. Plan of Correction: The Health Services Coordinator will notify via memo reminding staff that all medications are to be maintained in the original container with the pharmacy label attached.
Standard #: 22VAC40-185-550-M Description: Based on documentation reviewed, the Center failed to record all required information on the written record of children's minor injuries. Evidence: The LI reviewed a written record of a minor injury in the record of child #3, dated 09/04/15. How the parent was notified of the injury was not documented on the form. Plan of Correction: The Supervisor will address with all staff to ensure all information is completed on minor injury reports at the time of the incident.
