An unannounced monitoring inspection was conducted on Tuesday, September 29, 2015, from approximately 9:30am until 12:30pm. The following staff to child ratios were observed during today's inspection: ten infants with three staff; ten toddlers with two staff; Room 2A (two's) twelve children with two staff; Room 2B (two's) ten children with two staff; 3A (three's) nine children with one staff; 3B eight children with one staff; 4A (four's) six children with one staff; 4B eight children with one staff. Activities observed today included children practicing writing their names and letters, graphing sea creatures, painting, dancing, reading, and using a variety of manipulatives. A practice evacuation drill was conducted 9/21/15. AM Snack: Fresh fruit and milk Lunch: Macaroni and cheese, salad, peaches and milk Five staff records, six children's records and three medications were inspected.
Standard #: 22VAC40-185-70-A Description: Based on review of staff records, the center did not ensure all required items were documented in each staff record. Evidence: Out of five staff records inspected, Staff #1, hired 9/16/15, had a reference dated after employment, 9/21/15. Documentation of two references is required prior to employment. Staff #3, hired 4/13/15, is missing a second reference. Staff #3 does not have documentation of the education required as a Lead Teacher. Plan of Correction: Per Center: Follow-up more closely after orientation to make sure paperwork is complete. Getting second reference and requested documentation of education.
Standard #: 22VAC40-185-270-A Description: Based on inspection of the facility, the center did not ensure all areas were maintained in a clean and safe condition. Evidence: The window ledge in the toddler classroom has peeling paint. Plan of Correction: Per Center: Notified maintenance of peeling paint. This center location scheduled for painting.
Standard #: 22VAC40-185-510-N Description: Based on inspection of medications present at the center, the facility did not ensure authorizations had not expired.
Out of three medications inspected, Medication #3 authorization expired 9/10/15.
Plan of Correction: Per Center: Notified parent via email. Note on calendar to check meds monthly.
