Comments:
The inspector arrived at 9:30 AM and departed at 11:30 AM. Breakfast service and morning activities were observed. No children's injuries were reported for this school year. Medication administration was reviewed. A sampling of five children's records and three staff records were reviewed. An exit meeting was conducted with the program director during which the facility's Plans of Correction were obtained prior to concluding the inspection.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based upon review of five children's records, the facility has not ensured that they obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: The record for child 2, who began attending the facility on 9/8/2015 does not include documentation of immunizations.

Plan of Correction: The facility responded with the following: The Family Service Worker will get immunization record from parent today.

Standard #: 22VAC40-185-510-A
Description: Based upon review of medication to be administered, as needed, to children in care, the facility has not ensured that medication is administered according to the facility's written medication policy. Evidence: The facility's medication policy requires written parental authorization for medication to be stored for emergency administration to children. There was an epi-pen jr stored for potential administration to child 6. The facility could not produce written parental authorization for this medication to be administered.

Plan of Correction: The facility responded with the following: The Family Service Worker will get parent to sign permission slip to administer epi-pen jr.
