An unannounced monitoring inspection was conducted from 4:05 p.m. to 5:30 p.m. At the time of entrance the following adult to child ratios were observed:18 children, ages 5 to 11 years old, with 2 staff. The records sample size consisted of 4 children?s records. Staff records are housed off site and were recently reviewed in July of 2015 . Children were observed playing outdoors, having an afternoon snack which consisted of cherry strudel and orange/pineapple juice and completing homework assignments. Hand washing, table sanitizing and restroom procedures were also observed. Medication administration was not observed. Staff #3 stated that there are no children currently being administered medication while in attendance. Areas of non-compliance are identified on the violation notice. The licensee provided verbal responses to the violations during the inspection, which were entered on to the violation notice by the Licensing inspector.
Standard #: 22VAC40-185-540-C Description: Based on observation and interview the licensee did not ensure that the first aid kit contains all of the required components. Evidence: The first aid kit available on site did not contain a triangular bandage. When asked if a triangular bandage was available on site, staff #1 stated that a sling had been placed in the kit for this component and that she did not realize that it would not adequately substitute for a triangular bandage. Plan of Correction: Staff #1 stated that a triangular bandage would be purchased and placed in the first aid kit by tomorrow.
Standard #: 22VAC40-185-540-D Description: Based on observation and interview the licensee did not ensure that all required emergency supplies are available on site and on field trips. Evidence: The emergency supplies reviewed on site did not include a flashlight. In addition, the battery operated radio was not operable without electricity. When asked if these items were available, staff #3 was unable to locate a flashlight or batteries to operate the radio that was available on site. Plan of Correction: Staff #3 stated that a flashlight and batteries for the flashlight and available radio would be purchased and placed in a known designated location where staff could access them by tomorrow.
