Comments:
An unannounced monitoring inspection was conducted from 10:00 a.m.-11:45 a.m. with the center director. 41 children, ranging in age from 3-6, were being supervised by six staff. Children were observed working on jack-o'-lantern art and eating their morning snacks. The playground could not be inspected due to rain. The fire and health inspections are up-to-date. The last fire drill was 9/23/15. 17 staff are first aid/CPR certified; 18 staff are daily health observation (DHO) certified; and 1 staff is medication administration training certified. Three children's records were reviewed. Two staff records were reviewed and in compliance. Three medications were reviewed and in compliance. Standard #: 22VAC40-185-150-B
Description: Based on review of three children's records, the center did not have documentation of a child's physical and immunizations that were signed or stamped by a doctor, his designee, or an official of a local health department. Evidence: Child A's documentation of physical and immunizations were printed pages that were not signed or stamped by a doctor.

Plan of Correction: The center will notify the parent to get this signature from the doctor.
