A complaint investigation was begun at the facility on 09/03/2015 in response to a complaint received on 08/28/2015. Additional allegations were received on 09/04/2015. The investigation was continued at the facility on 10/01/2015. Observation, review of documentation and staff interviews did not substantiate the allegation of inadequate staffing and supervision of the children. Not valid. A review of staff documentation and qualifications revealed that a program director or back-up director was at the facility at least 50% of the hours of operation. Not valid. Observation and staff interview did not provide a preponderance of evidence that children are fed inadequate food portions without the availability of seconds. Not valid. Infant and toddler children were not observed to be confined in high chairs for periods to exceed 15 minutes. There was no observed evidence of insect infestation. not Staff training documents, observation and interviews revealed that behavior management techniques used are appropriate. A violation associated with this complaint is cited on the attached violation notice. An exit meeting was conducted with the program director at the close of the investigation on 10/01/2015. The facility's plan of correction was obtained during the exit meeting. Please contact the inspector if there are any questions.
Standard #: 22VAC40-185-560-F Complaint related: Yes Description: Based upon review of posted menu and interviews with staff, the facility has not ensured that when the facility provides meals and snacks, they follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Evidence: 1. Staff interviews indicated that for lunch on 08/28/2015, the children were served a fruit, a vegetable, string cheese and milk. This lunch did not include a component from the grain component. 2. The menus provided for the weeks of 08/24/2015 - 08/28/2015 and 08/31/2015 - 09/04/2015 indicated that several afternoon snacks served represented only one food group component. These listed snacks were confirmed by staff interviews. - 08/25/2015 - water and ritz (crackers) - 08/26/2016 - water and gold fish (crackers) - 09/02/2015 - water and vanilla wafers - 09/04/2015 - water and grahams (crackers) Plan of Correction: The facility responded with the following: - Menu has been corrected to include all components. - Menu posted in visible area of kitchen. Director/Assistant Director will be making/distributing meals. - Has been corrected since last visit on September 3, 2015.
