Comments:
Today an unannounced monitoring visit was conducted beginning at 12:40pm. There were 101 children present with 17 staff, the Director and Assistant Director. Children were observed napping during this inspection. Children have been learning letters, spanish words, and about 911 and fire safety. Classrooms are decorated with fall-themed crafts and projects. A new playground climbing structure has been added to the facility for preschool aged children. Children's records, staff records and medications were reviewed today. This inspection, including the exit interview, ended at 3pm. Should you have any questions, please call.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review, four staff records (#1,2,3,5) do not have updated TB test results dated within two years of the most recent results. Evidence: Documented most recent TB test results for staff are: 2/2013 for staff #1, 8/2013 for staff #2, 3/2013 for staff #3 and 5/2013 for staff #5.

Plan of Correction: We will send staff to renew TB tests immediately. We will update every two years for all staff.

Standard #: 22VAC40-185-70-A
Description: Based on record review, one staff record (#6) does not have required updated documentation in their file. Evidence: Staff #6 has no documentation that repeat background checks have been submitted and returned within 3 years of the most recent results. The last documented results for search of the central registry and criminal history background checks are dated February, 2012.

Plan of Correction: We did renew these for this staff person and I believe we sent them to corporate. We will ask for the original copies back or submit new ones.

Standard #: 22VAC40-185-260-A
Description: Based on discussion and record review, there is no documentation of an annual fire inspection since 10/3/13.

Plan of Correction: We have had two visits with fire inspections since the last licensing inspection. We did not receive a copy of the fire inspection but have another scheduled for October, 2015. We will ask for documentation of this inspection for our records.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not left in a locked place, inaccessible to children. Evidence: During nap time in the PreK 1 classroom, two cleaning supplies, a spray and wipes, were left on a table within children's reach and were not being used.

Plan of Correction: The cleaning supplies were locked away immediately. We will make sure they are locked away when we are not using them.

Standard #: 22VAC40-185-330-A
Description: Based on direct observation, the playground has a safety hazard. Evidence: On the playground in between the school age climbing equipment and the new preschool climbing equipment there is a plastic fence to separate the playgrounds by age. The plastic green fence has stakes in the ground holding it up. The stakes have sharp metal edges exposed at the top at eye level for some children.

Plan of Correction: We were using this to keep the playgrounds separated until our permanent fence is put in. This was removed from the playground today and staff will keep children separated if different age groups are using the playground structures at the same time. We expect our new fence to be installed any day now.
