Technical Assistance:
Consultation was given on the new Director attending Phase II for CDC's at the Fairfax Area Licensing Office on Oct. 15, 2015. If interested, please call 703-934-1505 to register. Reviewed VA DSS model medication forms on the web site.

An unannounced monitoring visit was conducted. Three classes were in session. Observed children on the outside playground, transition to hand washing, preparation for rest time and lunch. Food is bought from home. Reviewed 3 staff files, a sample of children's records, emergency preparation and care of children. All rooms were in ratio with direct care staff. Description: Based on record review, 2 of 3 staff records are incomplete. Evidence: Staff #1 and Staff #3 are missing reference documentation prior to employment.

Plan of Correction: Reference calls are documented.

Standard #: 22VAC40-185-510-C
Description: Based on medication review, the center has not kept current authorizations for 3 of 4 medications reviewed. Evidence: Written authorizations were older than one year for the following children: 1. Child #1's authorizations (parent and physician) for 2 medications expired 8/2015. 2. Child #3's authorization for one medication (parent-8/15/15 and physician 9/10/2015). 3. Child #4 's authorization for a medication expired 5/2015 for both parent and physician.

Plan of Correction: Authorizations have been updated.

Standard #: 22VAC40-185-510-E
Description: Based on observation, the medication for one child was not in the original container with identifying information. Evidence: Child #3's medication was not in the pharmacy box and did not contain name, dosage and time to be given.

Plan of Correction: Contacted child's parent who will bring in medication and action form.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on record review, staff #1 did not have a completed Child Protective Search (CPS) within 30 days of employment. Evidence: Staff member was hired January 2015 and did not have the CPS clearance on file or documentation by the center that had contacted DSS within after the 30 days to follow up on the search in the Central Registry.

Plan of Correction: CPS clearance obtained.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, an employee was not denied employment when a Central Registry Finding (CPS) was not on-site within the 30 day time frame and no follow up by the center was documented. Evidence: Staff #1 was hired in a sub position (January 2015) and a completed CPS check was not available. The back page was not completed by DSS with a signature and stamp. The employee has been working past the 30 days and has been consistently working at the center since January 2015.

Plan of Correction: CPS clearance obtained.
