An unannounced monitoring inspection was conducted today from 10:00am-11:30am. There were 63 children (infants-5yrs.) directly supervised by 11 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, and emergency supplies were inspected. Children were observed participating eating snack, participating in circle time, singing songs, and playing on the playground. There was an abundant supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A Description: Based on record review, two of four children's records reviewed were incomplete. Evidence: Documentation of an annual update of the child's information was not observed for Child 3 and Child 4. Plan of Correction: I will address today and create a system to make sure we are reviewing with parents.
Standard #: 22VAC40-185-70-A Description: Based on record review, one of four staff records reviewed was incomplete. Evidence: Documentation of only one of two required references was observed for Staff D, who was hired July 21, 2015. Plan of Correction: I will complete that today.
Standard #: 22VAC40-185-550-M Description: Based on injury record review, written records of children's injuries were incomplete. Evidence: 1. The time in which the parent was notified of the injury was not observed documented on all four injury records reviewed. 2. The method of how the parent was notified of the injury was not observed documented on three of four injury records reviewed. Plan of Correction: We will have staff meeting next Tuesday and I will review at the meeting. I will meet with the teacher who completed the reports today.
Standard #: 22VAC40-191-60-C-1 Description: Based on record review, the center did not obtain an original criminal history record report within 30 days of employment for staff. Evidence: Documentation of an original criminal history record report was not observed for Staff C, who was hired July 31, 2015. A photocopy of the form was observed in the staff's record. Plan of Correction: The form was mailed and has not returned. I will call about the form.
