Comments:
Today an unannounced monitoring visit was conducted with the Director-Mrs. Ilisa St. Pierre this visit took place in the morning between 9:30-11:30 am there were two classrooms observed with a total of 16 children with 3 staff within the supervision guidelines. The children were observed having morning circle time, painting, snack, free play, story-time and in transition for lunch and dismissal. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation of the physical plant, the following areas of the center were found not being maintained in a clean, safe and operable condition. Evidence: Upon arrival, rusting was found along the stairs/railing leading to the back door entrance .

Plan of Correction: Director stated that she contacted the landlord and is waiting for them to fix it.

Standard #: 22VAC40-185-560-G
Description: Based on observation of food brought from home, 5 lunch bags were found without the name and date identifying the owner. Evidence: 1 lunch bags did not have date.

Plan of Correction: Director stated that parent will be reminded to include the date with all lunch containers.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on observation of the staff records, one record did not include an updated criminal record check within three years from the date of the last screening. Evidence: Staff #1's record criminal record check was beyond the expiration date (8/12/15).

Plan of Correction: Director stated that she will make sure the background check is updated immediately.
