Comments:
An unannounced monitoring inspection was conducted from 9:53 a.m. - 12:40 p.m. There were forty-four children present with seven teachers working with them. Three children's records were reviewed and the record for one new staff person was reviewed. The children were observed working at the life cycle center, counting, pouring water and using a spoon to secure beans for in their cups, working with different shapes and sizes, identifying colors, painting with different brushes, engaged in discussion, exploring different cultures, hand washing, toileting, dancing and twirling, playing outside, and during lunch. The younger children were observed during hand washing, toileting, engaged in putting their shoes on to go outside and gathering for community time. Description: Based on a review of documentation the facility failed to have a record of an evacuation drill for the month of September 2015. Evidence: 1. On 10/08/2015 the licensing inspector reviewed the monthly evacuation log and there was no documentation of an evacuation practice conducted for the month of September 2015. 2. On 10/08/2015 staff #1 confirmed there was no record of an evacuation practice for September 2015.

Plan of Correction: We have conducted our evacuation practice for this month 10/07/2015 and will continue to do so on a monthly basis as required.
