Comments:
An unannounced monitoring inspection was conducted on October 8, 2015 during the after care portion of the school age program. During the inspection there were five children present with two staff members. There are no medications present or being administered by the program staff. The licensing inspector observed the children playing with various tabletop activities. Two children's records were reviewed.

Violations:
Standard #: 22VAC40-185-260-A
Description: Based on review and inspection of the facility, the center did not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection conducted on October 8, 2015, the fire inspection report provided to the licensing inspector had expired and was dated 10/1/2014.

Plan of Correction: We will continue to ask the school to notify us so we can obtain a copy.
