Comments:
An unannounced monitor inspection was conducted on October 8, 2015. Upon arrival, there were sixteen (16) children ages 5 months-4 years old, the Owner, Center Director & three (3) staff members present. Attendance in each group was as follows: Infant Classroom There were three (3) & one (1) staff member present. 2 Year Old Classroom There were seven (7) 2 year old children & two (2) staff members present. 3-4 Year Old Classroom There were five (5) 3-4 year old children & one (1) staff member present. Children & staff records reviewed including children medication & injury records. Center continues to administer medication & does not provide transport. Children were observed during rest time, arrival departure procedures, active/quiet activities, self-initiated and directed activities; bath room/diapering/hand washing procedures, meals/snacks served & nap time. Risk Assessment & areas of non-compliance found during today?s inspection were reviewed with the Program Coordinator on 10/8/2015.

Standard #: 22VAC40-185-270-A
Description: Based on inspection, the Licensee failed to ensure areas and equipment of the center are maintained in a clean, sage and operable condition. EVIDENCE: In the boys bathroom, LI Anderson observed a broken stainless steel toilet paper holder. A portion of the holder was removed from the wall & laying on the floor with 2 nails attached. The children in care have immediate access to this area and used frequently throughout the day.

Plan of Correction: Maintenance man will called out to fix. Broken portion removed during inspection.

Standard #: 63.2-1720-F
Description: Based on discussion with staff & record review, the Licensee failed to ensure employees without completed background checks are not permitted to work alone with children unless employee works under the direct supervision of another employee whom background checks has been completed in accordance with the requirements. EVIDENCE: Staff #3 (date of hire 8/24/2015) without completed background checks(Criminal Record Check), was allowed to care for children alone without direct supervision of another employee with completed backgrounds.

Plan of Correction: Staff will not be left alone without completed background checks. Corrected during inspection
