Comments:
An unannounced monitoring inspection took place today from 10 - 12:30 pm. There were five classes present today during the inspection with 73 children present. I observed children eating blueberry muffins they had baked this morning to extend their study of the book "If You Give a Moose a Muffin;" learning to say 'pumpkin' and other words in Spanish; singing; building with blocks and manipulatives; and playing outdoors. Violations:
Standard #: 22VAC40-185-270-A
Description: During today's inspection, some areas and equipment were observed not to be maintained in a safe condition and/or operable condition. Evidence: In one of the children's bathrooms, step stools to reach the sink were placed on bath rugs with minimal backing that appeared to have lost their gripping ability, posing a potential slipping hazard. On wooden play structures outdoors, there were some areas of splintering wood observed. The wooden slide outside the Dolphin's room was badly torn on the bottom rung. Peeling and chipping white paint was observed on the fence in the younger children's area. In the red barn, there was a rope hanging on a pole and a hanging electric cord from an light fixture, accessible while standing on the tire below it. A pedal was missing from tricycle carousel. The pads of resilient surfacing under swings and the former slide were up at ends, resulting in uneven surfacing, posing a potential tripping hazard.

Standard #: 22VAC40-185-280-I
Description: During today's inspection, one class was observed handling a craft material that was not intended for use with children. Evidence: Children were observed handling and exploring a large pan of craft material of tiny gel-like beads that expanded with the addition of water. Staff reported that a parent had brought in two packages of the craft material. The packages had a label that indicated it should be kept out of the reach of children and it posed a choking hazard.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 22VAC40-185-330-B
Description: One area of the playground was observed today to not have an adequate amount of resilient surfacing in the fall zone of a piece of equipment today. Evidence: The small framed area where the tricycle carousel was located had some bare spots with less than six inches of mulch in the area adjacent to the tricycles.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 22VAC40-185-510-N
Description: During a review of seven children's medications today, one child's long term medication was present but the authorization for it was observed to no longer be effective. Evidence: A long-term medication authorization for Child #5 expired 9/14/15, but the medication was still present at the center three weeks after the expiration .

Plan of Correction: Not available online. Contact Inspector for more information.
