An unannounced monitoring inspection was conducted on October 5, 2015. At the time of entrance there were 14 children and 2 staff members present. A sample size consisted of 4 children's records. Children were observed working on homework, playing with board games, dolls and Legos. Two medications were reviewed during the inspection. The inspection began at approximately 3:10 P.M. and ended at 5:15 P.M.. Areas of non-compliance were noted on the violation notice.
Standard #: 22VAC40-185-60-A Description: Based on a record review the licensee did not ensure all enrolled children had a record containing the required information. Evidence: During the inspection conducted on October 5, 2015, the following records were determined to be incomplete; 1) The record for child #1 did not include the employment phone number for both of the parent's listed or a copy of an immunization record placed in the child's record before the first day of attendance. Center staff confirmed first date of attendance was 9-21-15. 2) The record for child #2 did not include a copy of a physical conducted and placed in the child's record within one month of attendance. Center staff confirmed first date of attendance was 9-8-15. 3) The record for child #3 did not include the complete addresses for both emergency contacts listed. Plan of Correction: The center supervisor will contact each parent for the information to have a complete record on file.
Standard #: 22VAC40-185-560-G Description: Based on observation and staff interviews, the licensee did not ensure that when food is brought from home the containers are sealed and clearly dated and labeled in a way that identifies the owner. Evidence: During the inspection conducted on October 5, 2015, four children were observed during snack time eating food brought from home, four of the lunch boxes /containers were not labeled with a date and two of the lunch boxes/ containers were not labeled in a way that identifies the owner. Plan of Correction: The center supervisor will ensure all food brought from home is labeled each day with the current date /owner name.
