Technical Assistance:
1. The Supplemental Information page of the inspection report is not to be posted in the facility. 2. Medication authorization forms are to match the medicine exactly, to include: name of medicine and dosage. Any discrepancies are to be verified and corrected.

Comments:
Conducted an unannounced monitoring inspection. Observed 76 children + 13 direct-care staff. Ratios and supervision were in compliance. Staff were sitting and talking with children during lunch, assisting with handwashing as needed, and setting up cots for naptime. Children were observed washing hands, eating lunch, having story-time, and preparing to take naps. Lunch served: macaroni + cheese, peas, pineapple, and milk. Classrooms were found to be clean, safeguarded appropriately, and sufficiently supplied with toys and equipment for the children. The outdoor playground was in compliance. Medication management was not in compliance. Required documents were posted. Record keeping violations found. Inspection completed 11:15am-12:40pm. Standard #: 22VAC40-185-240-A
Description: Based on records reviewed, it was determined that 1 of 5 staff records did not include documentation of orientation training. Evidence: Staff #1-Hired in December 2011.

Plan of Correction: Include orientation training for staff member and include in files

Standard #: 22VAC40-185-510-A
Description: Based on medication records reviewed, it was determined that 2 of 4 medications did not have proper authorizations for the center to administer. Evidence: Child #1-(1) The authorization for an Epi-pen was written on a form that gave Fairfax County Public School personnel permission to administer the medicine, which was on-site. (2) The authorization for Benadryl was written on a form that gave Arlington County Public School + Dept. of Human Services personnel permission to administer the medicine, which was on-site.

Plan of Correction: Have parent return the form provided by the school instead of form provided by the physician

Standard #: 22VAC40-191-60-C-1
Description: Based on records reviewed, it was determined that 1 of 5 staff records did not include a completed Criminal Record Check (CRC). Evidence: Staff #9-Date of hire was in February 2014. The 30-day grace period has expired, and employment has not been denied. The staff person has worked in a direct-care position.

Plan of Correction: Re send Criminal Background Check for staff member and include in files.
