Arrived 8:05am Departed 11:15am An unannounced renewal licensing study was conducted today. Seventy-five children were present with 15 staff. The children were playing centers, drawing, singing, playing outdoors and eating lunch. Three staff files and five children's files were reviewed for compliance. Congratulations on the new playground equipment and windows! You reported that the silver metal climber is not in use and will not be used until it is repaired. Thank you for your cooperation. Standard #: 22VAC40-185-330-B Description: Based on a tour of the playground, the facility did not maintain minimum required depth of resilient surface material on the preschool playground. Evidence: 1. The measured depth of mulch at the base of the slide is 2-4 inches. Plan of Correction: The program director reported that mulch has been ordered for this playground, but due to rain it has not been delivered. It will be delivered and distributed tomorrow.
Standard #: 22VAC40-185-510-G Description: Based on the review of the medication log, MAT certificates and an interview with the program director, the facility did not follow medication administration requirements. Evidence: 1. Staff #3 administered an inhaler to child #5 on 7/7/15. 2. Staff #3 does not hold MAT certification. Plan of Correction: The program director reported that she will review medication procedures with staff #3 to insure that these procedures are followed every time medication is administered. Only MAT-trained staff will administer medication.
