Arrived 11:00am Departed 1:10pm An unannounced monitoring inspection was completed today. Nine infants and toddlers were present with three staff. Eleven twos were present with four staff. Five threes and fours were present with two staff. The children were playing in centers, having group time, eating lunch and resting. Thank you for your cooperation. Standard #: 22VAC40-185-160-A Description: Based on the review of one staff file, the facility did not require a TB test or screening within 21 of employment. Evidence: 1. Staff #1 was hired on June 5, 2015. There is no record of a TB test or screening in the file. Plan of Correction: The program director will insure that staff #1 has a TB test or screening.
Standard #: 22VAC40-185-330-B Description: Based on a tour of the playground the facility did not maintain six inches of resilient surface material around climbing equipment. Evidence: 1. The LI measured 2-4 inches of pea gravel at the base of two blue slides. Plan of Correction: The program director will request more pea gravel be ordered and distributed on the playground.
Standard #: 22VAC40-185-430-C Description: Based on observation, the facility did not maintain all equipment in a safe condition. Evidence: 1. The spindles on the new loft in the three's and four's room are allow a 3.5 in X 6.25 rectangle to pass through the opening, but do not allow a nine inch circle to pass through. This is head entrapment point. Plan of Correction: The camp director reported that he will adjust the spindles on the loft to no more than 3 1/2 inches distance apart.
Standard #: 22VAC40-191-60-C-1 Description: Based on an interview with the executive director, the facility did not obtain all background checks within 30 days. Evidence: 1. There are no criminal history record checks on file for adults #1, 2, 4. Plan of Correction: The camp director reported that the criminal record checks were completed and mailed. They have not been returned from the Virginia State Police.
Standard #: 22VAC40-191-60-C-2 Description: Based on an interview with the executive director, the facility did not obtain all background checks within 30 days. Evidence: 1. There are no CPS Clearances on file for adult #4. Plan of Correction: The camp director reported that the CPS Clearances were completed and mailed. They were returned because they were on the wrong form. New forms were completed have been resubmitted to the central registry.
