22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 8:00 am-12:00. At the time of entrance, 110 children were in care with 22 staff members present. Children were observed during snack time, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. Standard #: 22VAC40-185-130-B Description: Based on a review of children records, licensee has not obtained documentation of additional immunizations once every 6 months for children under the age of 2 years. Evidence: Child #1, date of birth 7/19/14 does not have documentation of updated immunizations. There was one immunization documented dated 9/29/14. Plan of Correction: Parents of child #1 were notified. They immediately had doctors office fax over most recent immunizations.
Standard #: 22VAC40-185-510-C Description: Based on a review of medications, licensee has not followed the procedures for administering medication by having an expired medication on cite. Evidence: Child #2 medication, Benadryl expired 9/2015. Plan of Correction: Parent of child #2 was contacted and requested was made to update medication authorization. New medication was given to school, expired med returned.
Standard #: 22VAC40-185-510-D Description: Based on a review of medications, licensee has not maintained medication authorization to be available during the entire time it is effective. Evidence: The medication authorization form for child # 3 expired on 6/25/15 and the medication, Benadryl and epi pen was still on site. Plan of Correction: Parents of child #3 came in and updated medication authorization. Medication and epi pen were also updated.
Standard #: 22VAC40-185-510-N Description: Based on a review of medication, licensee has not notified the parent within 14 days to pick up the medication or it will be disposed of by the center. Evidence: Child # 4 medication, Epi pen was found at the center without a current authorization form. Staff said that the child no longer attends the center. Plan of Correction: Medication of child #6 was appropriately disposed of.
Standard #: 22VAC40-185-550-D Description: Based on a review of fire drills, licensee has not implemented a monthly evacuation drill. Evidence: There was no documentation of fire drills for the month of September, staff acknowledged that there was no drill completed. Plan of Correction: Checks and balances system implemented so management can check upon each other to ensure monthly drills are completed.
