An unannounced monitoring inspection was conducted on October 20, 2015. There were 22 children present with two staff members. The children were observed doing homework, reading books and playing with other tabletop activities. The licensing inspector reviewed the physical plant, supervision, required postings, children's records, emergency evacuation and shelter-in-place documentation, and nutrition. The posted menu indicated that today's snack consisted of goldfish crackers and apple juice. There are no medications present or being administered in the program. Three children's records were reviewed during the inspection. It was observed that on the playground equipment utilized by the children in care, there were some areas of peeling paint and rust. The black plastic border surrounding the playground area had several bolts protruding that could potentially be an entanglement hazard.
Standard #: 22VAC40-185-260-A Description: Based on observation and inspection of the facility, the center did not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection conducted on October 20, 2015, the fire inspection report for the center had expired and was dated 10/01/14. Plan of Correction: Site Director has already spoken with an official from the school. A copy of the report will be obtained.
